From my hotel check out 12.00pm till my return flight time 7.10pm, can anybody suggest any activity in phuket? I'm traveling with my wife & my sons (3 years & 5 years old).

Seems like you will have about 3 to 4 hours to spare.
Go and relax at one of the beaches (Kata, Karon, Patong), etc if you dont mind getting wet. Otherwise go shopping at Juncelon Shopping Centre in Patong and enjoy a massage after that.
My Suggestion would be to ask the hotel for a late checkout. It is not always possible especially during peak season but if they can accommodate you then they will always try. Obviously you would not want to pay for a full day rate but if they did ask for something you may be able to negotiate a small charge, but from my experience most will allow a late checkout when available or at least allow you to use the hotel facilities and hold your luggage until you leave for your flight. With two young children I would think you are best to stay and the hotel and preferably with your room as long as you can before the flight.
